Acute Appendicitis: The Abdominal Pain You Should Never Wait Out

Of all the emergencies I treat, acute appendicitis is one of the most common — and one of the most time-sensitive. It can affect anyone, at any age, often with no warning and no clear cause. The frustrating thing about appendicitis is that it can start so quietly, feeling like nothing more than an upset stomach, that many patients wait far longer than they should before seeking help. This article explains what the appendix is, why it becomes inflamed, how we diagnose it accurately, and why acting quickly genuinely changes the outcome.
What the appendix is, and why it becomes inflamed
The appendix is a small, finger-like tube attached to the beginning of the large intestine, in the lower right part of the abdomen. It has no essential function that we depend on in adult life, which is precisely why removing it causes no lasting harm. Appendicitis develops when the inside of the appendix becomes blocked — often by a small piece of hardened stool, or sometimes by swollen lymphatic tissue following a viral illness. Once blocked, bacteria that are normally harmless multiply rapidly inside the trapped space, causing the wall of the appendix to become inflamed and swollen. If this process isn’t interrupted, the pressure inside continues to rise until the blood supply to the appendix wall is cut off, and the tissue begins to die — from there, perforation (rupture) can follow, spilling infected material into the abdomen.
Appendicitis can affect anyone at any age, but it is most common in the teens through the thirties, and there is no reliable way to predict who will get it — there is no proven link to diet, and it cannot be prevented through lifestyle choices. This is worth saying plainly because patients sometimes blame themselves, wondering what they ate or did wrong. In the great majority of cases, it is simply an unlucky blockage of a small tube, not a consequence of anything the patient could have avoided.
How it typically presents
The pattern of pain is one of the most useful clues in appendicitis, and it is often quite distinctive. Pain usually begins vaguely around the belly button — dull, crampy, hard to pinpoint — and over the following hours it shifts and settles into a more definite, sharper pain in the lower right side of the abdomen. This migration of pain, from the middle to the lower right, is one of the classic hallmarks of appendicitis, and it happens because early inflammation irritates general abdominal nerves, while later inflammation irritates the lining directly overlying the appendix, which is much more precisely localised.
Alongside the pain, most patients lose their appetite — often noticeably, to the point of feeling repelled by food — and nausea, occasional vomiting, and a mild fever are common companions. Some patients notice the pain worsens with movement, coughing, or going over bumps in a car, because the inflamed lining of the abdomen is sensitive to any jolt or stretch.
What we look for on examination
A focused physical examination remains central to diagnosing appendicitis, even in the era of modern scans. I specifically check for tenderness at a point called McBurney’s point — roughly one-third of the way between the front of the hip bone and the belly button — which corresponds to where the base of the appendix usually sits. Several other findings support the diagnosis: pain in the lower right when I press on the lower left side of the abdomen (Rovsing’s sign), pain when the right hip is rotated (an obturator sign), and pain when the right hip is extended (a psoas sign) — this last one is especially common when the appendix lies behind the colon, close to the muscle that flexes the hip.
Why appendicitis doesn’t always look “classic”
Not every patient follows the textbook pattern, and knowing this matters because an atypical presentation is exactly what leads to dangerous delays. In some people, the appendix sits tucked behind the colon rather than in its usual front-facing position — in this “retrocecal” position, the pain can be duller, felt more in the flank or back, and the classic tenderness at McBurney’s point may be far less obvious. In pregnancy, as the growing uterus pushes the appendix upward and outward, the pain of appendicitis can be felt much higher in the abdomen than expected — sometimes near the right upper abdomen by the later stages of pregnancy — which can understandably confuse the picture. In young children, the story is often harder to pin down simply because a child may not describe pain the way an adult does, and appendicitis in this age group can progress to perforation faster, partly because diagnosis is often delayed. In older adults, appendicitis is less common but tends to present later and more severely, sometimes with a more subtle fever and less dramatic pain despite significant underlying inflammation — this is one of the reasons I take abdominal pain in elderly patients particularly seriously.
Why the diagnosis isn’t always straightforward
Despite the classic pattern, appendicitis can be genuinely tricky to diagnose, and a fair number of other conditions can closely mimic it — gastroenteritis, urinary tract infection, and inflammatory bowel disease among them. In women of childbearing age, the differential is wider still, and I always make sure to consider gynaecological causes such as ovarian cysts, ovarian torsion, or pelvic infection, since these can look almost identical to appendicitis on initial assessment. For this reason, a full history — including a menstrual and pregnancy history in women — and a proper examination are never skipped, regardless of how classic the pain sounds over the phone.
How we confirm the diagnosis
Blood tests typically show a raised white blood cell count, reflecting the body’s response to infection, though this alone is not enough to confirm appendicitis. Doctors often use a structured scoring system — combining the pattern of pain, loss of appetite, nausea, examination findings, fever, and blood results — to judge how likely appendicitis is and whether surgery should proceed directly or further evaluation is needed first.
When the picture is classic, particularly in a young man, we can often proceed straight to surgery without needing a scan. When the diagnosis is less clear-cut — which is more often the case in women, given the wider range of possible causes — imaging becomes valuable. Ultrasound is quick, avoids radiation, and is often the first choice, especially in younger patients and in pregnancy. CT scanning is more accurate still — studies show CT correctly identifies appendicitis around 94% of the time, compared with roughly 83-88% for ultrasound — and is particularly useful when the presentation is delayed or atypical, though we use it thoughtfully because of the radiation exposure involved, especially in children. In practice, the choice between the two is individualised: a clear ultrasound finding can be enough to proceed, while an inconclusive ultrasound in someone whose story still strongly suggests appendicitis is a good reason to move on to a CT scan rather than risk missing the diagnosis.
Why waiting matters so much
This is the single message I most want patients to take from this article: appendicitis is not a condition to observe at home and hope improves. Once the appendix is significantly inflamed, it does not resolve on its own, and the risk of perforation rises the longer it is left — this typically begins to happen roughly a day or two after symptoms start, though the exact timing varies from person to person. A delayed presentation — several days of pain, high fever, a racing heartbeat — raises the likelihood that perforation has already occurred, sometimes with a collection of pus (an abscess) or a walled-off area of inflammation (a phlegmon) forming around the ruptured appendix. At that stage, the treatment path becomes more complicated, and the risks and recovery time both increase.
Treatment: surgery is the standard of care
For straightforward, non-perforated appendicitis, surgical removal of the appendix — an appendectomy — remains the standard, reliable treatment. This can be performed either through a single small incision in the lower right abdomen (open appendectomy) or through several tiny incisions using a camera and instruments (laparoscopic appendectomy). I generally favour the laparoscopic approach where appropriate: it typically means less pain after surgery, a faster return to normal activity, and a lower rate of wound infection. It also allows me to look around the rest of the abdomen if the appendix turns out to be normal, to find whatever else might be causing the pain — sometimes a gynaecological problem, sometimes inflammation elsewhere in the bowel.
For patients with uncomplicated appendicitis, a single dose of antibiotics around the time of surgery is all that’s needed afterward — there is good evidence that continuing antibiotics for days afterward does not reduce infection rates, and instead increases the risk of side effects like antibiotic-associated diarrhoea. Most patients go home within a day or two and are back to normal activity within one to two weeks.
When the appendix has already ruptured
Perforated appendicitis needs a more individualised approach, and immediate surgery is not always the right first step — in fact, operating straight away on a heavily inflamed, ruptured appendix with a large abscess can sometimes cause more harm than good, occasionally requiring a much bigger operation than anyone wants if the surrounding bowel is also involved.
When there is a well-formed abscess, the usual approach is to treat it first with intravenous antibiotics and, where needed, drainage of the abscess using a small tube placed with the guidance of an ultrasound or CT scan — this controls the infection without immediately operating into inflamed, fragile tissue. Once the inflammation has settled, typically after six to eight weeks, an interval appendectomy can be performed as a planned, much simpler procedure. Not every patient needs this later operation — for many, once the infection has fully resolved, the risk of the problem recurring is low enough that watchful monitoring is reasonable, particularly in younger patients with no other concerning features. I do recommend it for patients with recurring symptoms, a persistent lump of scar tissue, or any finding on the scan that raises concern about a growth rather than simple appendicitis — and in patients over 40, or with any suspicious features, a colonoscopy afterward is a sensible step to make sure nothing else is being missed.
Occasionally, a patient with perforated appendicitis becomes progressively more unwell — spreading abdominal pain, worsening fever, signs the infection is not staying contained — and in that situation, prompt surgery is the right call regardless of how much inflammation is present, because delaying further would be more dangerous than operating into a difficult field.
A rare surprise: tumours found in the appendix
Very occasionally, what looks like a routine appendectomy turns up an unexpected finding — a small tumour within the appendix itself, discovered either during the operation or on the pathology report afterward. These are uncommon, and the most frequent type is a small, usually well-behaved neuroendocrine (carcinoid) tumour. For small tumours, simple removal of the appendix is all that’s needed and nothing further follows; for larger ones, or if the report shows a different, more aggressive tumour type, a second, more extensive operation to remove part of the adjacent colon may be recommended. I always discuss the pathology report with patients afterward for exactly this reason — it occasionally changes what happens next, even when the operation itself went smoothly.
What to expect after surgery
Recovery from an uncomplicated, timely appendectomy is usually quick and straightforward — most patients are eating normally and walking within hours, pain is manageable with simple painkillers, and many go home the same or next day. The small keyhole incisions from laparoscopic surgery heal quickly, and most people are back to desk-based work within a week and to full normal activity, including exercise, within two to three weeks.
Recovery after surgery for a ruptured, complicated appendix takes longer — patients may need a few extra days in hospital, sometimes with a drain left in place temporarily, and a course of antibiotics continuing at home. Afterward, it’s worth knowing the warning signs of a problem developing during recovery: fever, worsening abdominal pain rather than steady improvement, nausea and vomiting returning after they had settled, or diarrhoea — any of these should prompt a call to your surgeon rather than waiting it out, since they can indicate a residual pocket of infection. With that said, the large majority of patients, including those with a ruptured appendix, recover fully with no lasting effect on digestion, fertility, or general health. The appendix truly is dispensable — once it is out, there is nothing left behind to cause future problems from that same source.
Warning signs you should never sit on
- Abdominal pain that starts near the belly button and moves to the lower right side over several hours
- Loss of appetite, especially alongside abdominal pain
- Nausea, vomiting, or fever accompanying abdominal pain
- Pain that worsens with movement, coughing, or going over bumps in a vehicle
- Abdominal pain lasting more than a few hours that is getting steadily worse rather than better
- High fever, a racing heartbeat, or spreading abdominal pain in someone who has had symptoms for several days — this can indicate the appendix has already ruptured
What you should do
If you or a family member develops pain that fits this pattern, please don’t wait to “see if it passes” — go to a hospital or see a surgeon the same day. Appendicitis is genuinely time-sensitive: patients who are assessed and treated early have a straightforward operation and a quick recovery, while those who delay risk a ruptured appendix, a longer hospital stay, and a more complicated recovery. When in doubt about abdominal pain, it is always better to be examined and reassured than to wait at home hoping it resolves.
