Acute Pancreatitis: Sudden, Severe Abdominal Pain You Should Never Ignore

Acute pancreatitis is one of the more dramatic emergencies in general surgery — a condition where a normally quiet, hidden organ suddenly becomes intensely inflamed, causing severe pain and, in a meaningful minority of cases, genuine danger to life. Most patients recover fully with the right treatment, but a small proportion go on to develop serious complications, and the difference between a straightforward recovery and a prolonged, difficult illness often comes down to how quickly the condition is recognised and properly managed. This article explains what acute pancreatitis is, why it happens, and what treatment actually involves.

What the pancreas does, and what goes wrong

The pancreas sits deep in the upper abdomen, behind the stomach, and does two essential jobs: it produces powerful digestive enzymes that break down food in the small intestine, and it produces insulin and other hormones that control blood sugar. Ordinarily, the digestive enzymes are made in an inactive form and only “switch on” once they’ve safely left the pancreas and reached the intestine. In acute pancreatitis, something disrupts this safety mechanism, and the enzymes become activated while still inside the gland itself. The result is that the pancreas essentially begins to digest itself — causing swelling, inflammation, and in more severe cases, tissue death (necrosis) and bleeding within and around the organ. This self-digestion is also why the inflammation can spill over into a body-wide reaction affecting the heart, lungs, and kidneys in severe cases, rather than staying confined to the abdomen.

Acute versus chronic pancreatitis — not the same condition

It’s worth clarifying a common point of confusion. Acute pancreatitis, the subject of this article, is a sudden inflammatory emergency that in most patients resolves completely once treated, though it can recur if the underlying cause isn’t addressed. Chronic pancreatitis is a different, separate condition — a long-term, often lifelong process where the gland gradually develops permanent scarring, typically from years of heavy alcohol use or, less commonly, repeated attacks of acute pancreatitis, leading to ongoing pain and, over time, loss of the gland’s digestive and hormonal function. Some patients with chronic pancreatitis do experience painful flare-ups that resemble an acute attack, which is one reason the two conditions are sometimes confused, but they are managed quite differently and deserve their own separate discussion.

What causes it

Two causes account for the large majority of cases I see. Gallstones are responsible for roughly half to two-thirds of cases — a small stone passing out of the gallbladder can temporarily block the shared drainage point where the bile duct and pancreatic duct meet, triggering the inflammatory cascade described above. Alcohol accounts for around a quarter of cases, through a combination of direct toxic effects on the gland and other contributing factors.

A longer list of less common causes exists, and I always try to identify a firm cause rather than simply labelling a case “unexplained” — this list includes certain medications (including some steroids and a few other specific drugs), high triglyceride (blood fat) levels, high blood calcium, complications following ERCP (an endoscopic procedure on the bile duct), abdominal trauma or recent abdominal surgery, some viral infections, and rare hereditary forms. With thorough investigation, genuinely unexplained cases should make up a small minority — if a clear cause isn’t found on first assessment, it’s worth continuing to look rather than settling for “idiopathic.”

How it typically presents

Pain is the hallmark symptom, and it has a fairly recognisable character: it comes on quickly, reaching its worst within minutes to a couple of hours, is severe and constant, and often doesn’t respond well to ordinary painkillers. It’s usually felt in the upper abdomen, sometimes concentrated to one side, and in about half of patients it radiates straight through to the back. Many patients instinctively find some relief by sitting up and leaning forward, and worsening pain lying flat. Nausea and repeated vomiting are common and can persist even once the stomach is empty. Because the pain and its radiation can mimic several other serious conditions — a perforated ulcer, gallbladder disease, or even a heart attack when it radiates to the chest — acute pancreatitis is a diagnosis that always needs to be actively considered rather than assumed away, and proper assessment in a hospital setting is the only reliable way to sort out which of these is actually happening.

On examination, most patients look unwell but stable; a smaller number appear genuinely gravely ill from the outset, with a fast heart rate, low blood pressure, and confusion reflecting the body-wide inflammatory response. Very occasionally, bleeding tracking through the tissue planes of the abdominal wall produces a bruised, bluish discoloration around the flanks or the belly button — a striking but uncommon sign that, when present, points toward a more severe, haemorrhagic form of the disease.

Mild versus severe — why the distinction matters so much

The great majority of patients — roughly 9 in 10 — have a mild attack: the pancreas becomes swollen and inflamed but there is minimal disruption to the rest of the body’s organs, and with supportive treatment, most people recover within about a week with a mortality risk of only around 1%. The remaining 1 in 10 or so develop a severe attack, where areas of the pancreas actually die off (necrosis) and the inflammatory response spreads to affect breathing, circulation, kidney function, or more than one of these together. This is a genuinely serious situation — mortality in severe pancreatitis has historically ranged from 20% to 50%, though outcomes have improved considerably with modern intensive care. Because it isn’t always obvious at the very start which category a patient will fall into, every patient admitted with acute pancreatitis is closely monitored over the first 48–72 hours, with specific scoring systems and blood tests used to flag early who is heading toward a severe course and needs a higher level of care.

In practice, this means a patient admitted with acute pancreatitis is reassessed at set checkpoints — on admission, at 48 hours, and again at one week — using a combination of blood tests (including markers of inflammation and organ function) and structured scoring systems that doctors have used for decades to predict which patients are heading toward a severe course. None of these tools is perfect on its own, which is exactly why they’re used alongside close clinical observation rather than in isolation — a patient who looks increasingly unwell is taken seriously regardless of what a score suggests.

How the diagnosis is confirmed

Diagnosis rests on a combination of the clinical picture and blood tests — specifically, a blood level of a pancreatic enzyme (amylase or, even more reliably, lipase) at least three times above the normal range, alongside the characteristic pain pattern. It’s worth knowing that a normal enzyme level doesn’t completely rule out pancreatitis, particularly if a patient presents some days after the pain began, which is one of several reasons the diagnosis sometimes needs a scan to confirm it rather than blood tests alone. An ultrasound scan is usually done early in every patient, less to diagnose the pancreatitis itself and more to check specifically for gallstones as the underlying cause and to look at the bile duct. A CT scan isn’t needed in every mild case, but becomes important when the diagnosis is genuinely uncertain, when a patient is deteriorating, or to assess the extent of any pancreatic necrosis in a more severe attack.

How it’s treated

For a mild attack, treatment is largely supportive: intravenous fluids to keep the patient well hydrated, proper pain relief, anti-sickness medication, and close observation, with eating restarted as soon as it’s comfortable to do so — there’s actually little benefit to prolonged fasting once nausea has settled, and modern practice favours resuming a normal diet earlier rather than later. Antibiotics are not needed for a straightforward mild attack, since this is an inflammatory process rather than an infection at this stage.

For a severe attack, the intensity of treatment increases considerably: admission to a high-dependency or intensive care setting, more aggressive intravenous fluid replacement guided by careful monitoring, supplemental oxygen, and close tracking of blood counts, kidney function, calcium, and blood sugar. If a patient isn’t able to eat normally for an extended period, feeding through a tube into the stomach or small intestine is preferred over intravenous nutrition alone, since keeping the gut working appears to reduce the risk of complications. Surgery has no role in the initial days of a severe attack — the priority is stabilising the patient medically — and is reserved for later, specific complications rather than the acute inflammation itself.

When gallstones are the underlying cause and there are signs the bile duct remains blocked — jaundice, or evidence of infection in the bile duct (cholangitis) — an urgent endoscopic procedure (ERCP) to clear the duct is carried out promptly, typically within three days, since this reduces the risk of further complications. Importantly, if gallstones caused the attack, removing the gallbladder is not something to defer indefinitely: current best practice is to do this during the same hospital admission once the patient has recovered from the acute attack, because leaving the gallbladder in place carries a real risk of a repeat, potentially more severe, attack of pancreatitis while waiting for a “later” operation that keeps getting postponed.

Complications that can develop later

Even after the initial dangerous phase passes, a subset of patients — usually those who had a more severe attack — go on to develop local complications over the following weeks. Fluid can collect around the inflamed pancreas; areas of dead pancreatic tissue can become walled off into a defined collection; and, most seriously, this dead or collected tissue can become secondarily infected, which is a situation carrying substantial risk and usually needing drainage, sometimes performed through minimally invasive radiological or endoscopic techniques rather than open surgery wherever possible. This is exactly why patients recovering from a severe attack are followed closely even after leaving the intensive care unit, with repeat scans if pain persists, fever develops, or blood tests suggest a new problem is brewing.

What recovery looks like

Most patients with a mild attack of gallstone or alcohol-related pancreatitis recover fully within a week or so and go on to live entirely normal lives afterward, provided the underlying cause is addressed — gallbladder removal for gallstone pancreatitis, and genuine, supported abstinence from alcohol where that was the trigger. Recovery from a severe attack, particularly one involving pancreatic necrosis, is understandably longer and sometimes involves further procedures for the local complications described above, but even patients who have a difficult initial illness frequently go on to make a good long-term recovery once the acute crisis has passed.

Warning signs worth going to hospital for immediately

  • Sudden, severe upper abdominal pain, especially if it radiates through to the back
  • Abdominal pain accompanied by persistent nausea or vomiting
  • Severe abdominal pain that doesn’t ease with rest or standard painkillers
  • Abdominal pain combined with fever, a racing heartbeat, or feeling generally very unwell
  • A known history of gallstones with new, severe abdominal pain
  • Heavy alcohol use with new, severe abdominal pain

What you should do

Sudden, severe abdominal pain of this kind is not something to wait out at home with painkillers — it needs same-day hospital assessment, with blood tests and, if needed, imaging to confirm the diagnosis and check severity early.
